Youth Services Survey (YSS) - November 13-17, 2017
The Department of Health Care Services (DHCS) is continuing its evaluation system for Children, Youth and Families Behavioral Health Services (CYFBHS). The collection of measures will occur once a year, during a one-week period. Information will be collected from parents/caregivers and youth (13 yrs and older) on satisfaction with services and service impact.
The assessment tools, which are the Youth Services Survey for Youth (YSS-Y) and the Youth Services Survey for Families (YSS-F), were chosen by the state and are to be completed on every child that receives services during the data collection period. All mental health services are included except for inpatient services, juvenile hall services, juvenile justice camp services and crisis services. All of the items are to be completed directly by the parent/caregiver or youth.
Administered to all parents/caregivers (YSS-F in the White cover sheet for English and Goldenrod cover sheet for Spanish) regardless of the youth's age AND youth 13 years and older (YSS in the Blue cover sheet for English and Purple cover sheet for Spanish) who receive billable services during the YSS period.

Fill in the program information BEFORE giving the packet to the youth/parent on the bottom of the form pages as well as the envelope:
- CSI County Client Number (Client's ID number - same number used in CCBH)
- Date survey was administered
- Reason not completed (if applicable)
- Form type (envelope only)
Please make sure respondents have PRIVACY when filling out forms and upheld at the utmost importance and confidentiality. Surveys should be returned to the program in the sealed envelope and not be opened by staff.
